How to complete the questionnaire

It is possible for you to complete sections of the questionnaire and save your work to allow you to fill in details at your convenience.

For example section E requests financial information which you may wish to ask another member of your organisation to complete.

If you can not respond to a question then please leave blank but please complete as many questions as possible.

If your organisation is part of a group structure or a branch of a larger organisation, please answer the questions on behalf of your part/branch only.
